On July 11, 1991 at approximately 8:30 AM, an employee was removing an old air supply line when the employee noticed that the new air supply line (1 1/2 inch 100psi) contained an air leak. When the employee attempted to tighten the 1 1/2 inch union the airline separated and began to move to and fro 10" to 12". This caused the employe e to fall from the machine where he was standing while working. The employee fell from a height of 46 inches, struck a four wheel cart, and died. Among other methods one feasable and acceptable abatement to correct this hazard is to secure the 2 inch and 1 1/2 inch air supply line against severe movement by installing permanent hangers, supports and anchors fabricated from durable, combustible materials and designed and installed in accordance with good engineering practices for the service conditions involved.
