29 CFR 1910.178(l)(3)(ii)(I): Powered industrial truck operators did not receive initial training in other unique or potentially hazardous conditions of the workplace that could affect safe operation: a) Dayton Parts LLC1300 North Cameron Street warehouse - The employer did not ensure that each employee completed training related to the Hubtex side lift forklift identifying the unique hazards associated loading and unloading from a side load forklift with restricted aisle space and retrieving additional stock from the racking, on or about August 31, 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
