Text For Citation: 01 Item/Group: 001 Hazard: BURNS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) At the AOD vessel located in the melt shop; The employer did not fully develop a preventative maintenance program that included written brick lining inspection criteria to reduce the potential for brick lining erosion and failure leading to molten metal breakthrough which may result in burns and/or explosions. This condition existed on or about April 4, 1993. A feasible and effective method of reducing the potential hazard would include but is not limited to: Assuring that a written preventative maintenance program is fully established and implemented to include written inspection criteria of the brick lines to determine its safe service life prior to charging.
