Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: hydrocarbon fire and or explosion hazards from the rupture or failure of pressure vessels, exchangers, and/or piping due to inadvertent closure of a block valve(s) upstream and/downstream of a safety relief valve. Safety relief valves with block valves located as described above include, but not limited to the following: (a) The pilot relief valve located at the Isomax vessel 12v3, Stage I Separator. (b) The pilot relief valve located at the Isomax vessel 12v7, Stage II Separator. Among others, a feasible and useful method to correct these hazards is set forth below: Install "car seals" or use locks and chains to prevent valves from being inadvertently operated as specified in API Recommended Practice 520 or Section VIII, Appendix M, of the ASME Code.
