Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by falling debris and hit by flying objects due to controls rooms that were not capable of resisting an unconfined vapor cloud explosion or located within the critical radius of such an explosion: a)Isomax control room. b)HF-Alky control room. c)Unifiner control room. Among others, one feasible method of abatement to correct this hazard includes, but is not limited to: 1)Following guidelines for plant layout and spacing. Such guidelines may include: a.The Dow Chemical Company "Fire and Explosion Index Hazard Classification Guide." b.Factory Mutual Loss Prevention Data Sheet, "Spacing of Facilities in Outdoor Chemical Plants." 2)If adequate spacing cannot be achieved, alternative methods of protection must be implemented such as explosion resistant construction or adequate barricading.
