29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section: a) Colorado Timber Resources, LLC at 210 County Road 21, Parshall, CO 80468: On or before July 24, 2015, the employer did not ensure that an employee was protected from being caught in-between the lift arms of the Bobcat skid loader and its frame. An employee was performing maintenance on the Bobcat skid loader, model 763 serial number 512243023, with the lift arms raised and without blocking them. When the lift arms lowered, the employee was caught in-between the lift arms and the Bobcat frame, with the resulting injuries contributing to this fatality. Conditions exposed employees to caught in-between hazards. b) Colorado Timber Resources, LLC at 210 County Road 21, Parshall, CO 80468: On or before July 24, 2015, the employer did not clearly and specifically outline the procedures for shutting down, isolating, blocking, and securing machines and equipment to control hazardous energy, and for the placement, removal, and transfer of lockout and tagout devices, and for testing the machines and equipment to determine and to verify the effectiveness of lockout and tagout devices. The lack of LOTO procedures applies to the following equipment: 1) the Cut Up and Saw Mill line, including such equipment as the cut-off saw, the log and waste conveyor, the debarker, the in-feed to the Scragg, the Scragg and vertical edger, the rough board stacker the chipper system, the resaw system, the board edger system, and the air compressor; and 2) the Planer line, including such equipment as the breakdown hoist, the trimmer system, the planer in-feed, the planer, the grading system, and the finished board stacker. Conditions exposed employees to amputation and electrical hazards.
