Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious harm to employees in that: a) Employees were exposed to being struck by the release of molten metal in that the design and manufacture of mold number 15, as per Centrifugal Casting Machine Company drawings D-13604 and D-13606 for Johnstown Corporation was not performed properly. Both the minimum diameter and the maximum diameter of the threads taped in the bottom of the mold wall were exceeded resulting in a loose fit. One feasible method of abatement would be to use acceptable and proper engineering quality controls procedures.
