29 CFR 1926.55(a): Employee(s) were exposed to material(s) at concentrations above those specified in the Threshold Limit Values of Airborne Contaminants for 1970 of the American Conference of Governmental Industrial Hygienists: On or about June 4, 2015, at a construction site located at 6450 South Hwy. 360, Grand Prairie, TX 75052, an employee working in a construction trailer was exposed to airborne concentrations of carbon monoxide which exceeded the OSHA 8-hour time-weighted average of 50 ppm. The overexposure to carbon monoxide resulted in the death of the employee.
