29 CFR 1910.110(b)(8)(iii): In a system where the gas in liquid form without pressure reduction entered a building, the employer failed to ensure that only heavy walled seamless brass or copper tubing with an internal diameter not greater than three thirty-seconds inch, and a wall thickness of not less than three sixty-fourths inch was used: (a) On or about June 15th, 2015, and times prior, at 467 Austin Avenue in Neosho, MO, employees were exposed to fire and explosion hazards in that the connection of two liquefied petroleum gas cylinders, designed for liquid withdrawal, were connected to the plant vapor piping system by use of synthetic flexible hoses without the use of approved pressure regulators. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
