Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of crushing, broken bones, severe lacerations and possible death. (a) On or about Oct. 27, 1992 employees were exposed to the hazards os crushing, broken bones, severe lacerations and possible death when loading or unloading rolls of ply rubber from the journal of the decker where pin locks and hair pin clips were not secured to the journal. Employees were exposed to injuries should the roll of rubber fall out of the journal. 1. Install cradles or saddles underneath each journals and pin locks and clips. 2. All pin locks will be painted bright red for better visible from floor level. 3. All chips will be attached by a small chain to the pin locks as a rewind to insert clips. 4. All employees will stand at floor level before releasing the hand break.
