The alleged violations below (2a -2b) have been grouped because they involv e similar or related hazards that may increase the potential for injury resulting from an accident: Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Employees were exposed to crushing injuries, broken bones, and death when required to perform maintenance, adjustment and repair on or in close proximity to, gear box and bulk material belt conyeyor while such equipment was capable of being acutuated. This pratice exposed employees to contact with moving belts, pulley, gears, rotating shaft and moving bulk materials. The lack of backstops, anti- runway or brake safety device materials were not provided as described by ASME B 20.1-1990, section 5.5. Among other methods, one feasible and acceptable method of abatement is to install backstops or anti-runaway safety devices to stop a dechning conveyor due to mechanical or electrical failure described by ASME B 20.1-1990, section 5.5.
