Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Pegasus Ellenburger #13-3: On or about August 21, 1989, a defective 2-3/8 inch pipe nipple and unsecured discharge line was used to release high well pressure as part of the "injection well" operation. The reverse unit operator was exposed to being struck by a blown out nipple and expanding (sweet) natural gas. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use tested pipe fittings and secure line with safety chain.
