Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fire and explosion hazard: a.On or about May 12, 1995, at about 3:13 p.m., and inside of Building 1734, Roosevelt Roads Naval Station, Ceiba, P.R., the employer was engaged in fueling operations of target BQM-34S, Serial 24367, while inside a building contrary to fueling instructions contained in Section 2-4 of NAVAIR 01-100TBA-2-3, (Department of Defense Publication) which requires that "the target must be fueled only when it is out-of-doors, and a minimum of 50 feet from buildings or equipment not engaged in servicing". Section 2-4 also contains the proper static ground cables (bonding) sequence connection between the refueler (transfer hose) and the target. The stated sequence for static ground (bonding) connection is as follows: (1) Target to ground, (2) Refueler to ground, (3) Refueler to target. Among others, one feasible and acceptable abatement method to correct this hazard is to conduct the fueling operations in accordance with the specific instructions contained in NAVAIR 01-100 TBA-2-3 (Department of Defense Publication).
