OSH ACT of 1970, Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: Jobsite located at 158 Cronin Avenue, Forestville, Connecticut: On, or about, May 4, 2015, an employee was instructed to use a portable step ladder to reach the top of tank trailer 96545 in order to open/close the vent hatch. The employee had to reach over the side of the ladder in order to perform the task.
