Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazard: (a) On or about April 10, 1991, at the work over site, employees were exposed to crushing hazards from being struck by a falling joint of oil well tubing which was released by the elevator on the pulling rig as the joint was being laid out in the tubing rack. Among other mechods, two feasible and acceptable abatement methods to correct this hazard are to: 1) Provide a secondary means of supporting oil well tubing as it is being raised and lowered above the well head; or 2) Use an elevator that is designed to prevent the unintention- al opening of the elevator.
