29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: a) The employer did not train its elevator mechanics whom it knew entered elevator pits that were permit-required confined spaces to perform servicing and maintenance-to have the understanding, knowledge, and skills necessary to evaluate and enter permit-required confined spaces. On 5/26/2015, at Iroquois Nursing Home in Jamesville, New York, an employee was fatally crushed by an elevator while preparing to enter an elevator pit (which was a permit required confined space) to make a repair. The employee was not trained to follow confined space procedures while working in the elevator pit.
