29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations including, but not limited to; specifying acceptable entry conditions, isolating the permit space, and verifying that conditions in the permit space are acceptable for entry throughout the duration of an authorized entry: a) Iroquois Nursing Home, Jamesville NY, First floor, Elevator Car #1, on or about 5-26-15: An employee was preparing to perform servicing and maintenance in an elevator pit that was a permit required confined space, but the employer did not have procedures and practices implemented to specify acceptable entry conditions, exposing the employee to a struck by hazard. b) Iroquois Nursing Home, Jamesville NY, First floor, Elevator Car #1, on or about 5-26-15: An employee was preparing to perform servicing and maintenance in an elevator pit that was a permit required confined space but the employer did not have procedures and practices implemented to ensure isolation of the permit space, exposing the employee to a struck by hazard. c) Iroquois Nursing Home, Jamesville NY, First floor, Elevator Car #1, on or about 5-26-15: An employee was preparing to perform servicing and maintenance in an elevator pit that was a permit required confined space but the employer did not have procedures and practices implemented to ensure that the conditions in the permit space were acceptable for entry throughout the duration of the authorized entry, exposing the employee to a struck by hazard.
