Text For Citation: 01 Item/Group: 003 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employement and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falls of over 20 feet: (a) The weight of the personnel platform was not posted and the maximum intended load permitted in the personnel platform was also not posted. The personnel platform, the use of which is normally prohibited, except when the erection, use, and dismatling of conventional means of reaching the work location, such as by a ladder, stairway, aerial lift or scaffold, would be more hazardous or is not possible because of work conditions, was suspended from the load line of the link belt truck crane HTC-850 in the Sulphuric Acid Plant, on or about 10/25/91. Among other methods, one feasible and acceptable abatement method to correct this hazard is that the personnel platform shall be conspicuously posted with a plate or other permanent marking which indicated the weight of the platform and its rated load capacity or maximum intended load. (b) Trial lifts, when made, were not done with the personnel platform loaded at least to the anticipated liftweight. The personnel platform, the use of which is normally prohibited, except when the erection, use and dismantling of conventional means of reaching the work location, such as by a ladder, stairway, aerial lift or scaffold would be more hazardous, or is not possible because of worksite conditions, was suspended from the load line of the Link Belt truck crane HTC-850, in the Sulphuric Acid Plant. Among other methods, one feasible and acceptable abatement method to correct this hazard is to make a trial lift with the unoccupied platform loaded at least to the anticipated liftweight from the location where employees would enter the platform to each location at which the personnel platform was to be hoisted and positioned.
