Text For Citation: 02 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or before April 23, 1991, employees were required and permitted to position themselves between the mast and fixed parts of the mast transporting trailer for the purpose of placing hinged mast support(s) under a partially elevated mast. These employees were exposed to the hazard of being struck and crushed by the mast as a result of a failure of the hydraulic lift system for the mast. One feasible and acceptable abatement method to correct this hazard is: 1) To insure that employees are not in a position to be struck by or caught between the mast and any fixed or temporary objects during the erection operation. 2) Provide the following equipment such as, but not limited to: ladder(s), step(s), platform(s), or a powered hoisting system that raise hinged mast supports. 3) Instruct employees performing this operation in the safe procedures for accomplishing this task.
