29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about April 3, 2015, at 585 Jackson Lake Road, Monticello, Georgia: Two employees performing roofing work using an approximately 35-foot tall shingle ladder with the base of the ladder set within three feet of an approximately 14,400 volt overhead power line were electrocuted when the employer did not train the employees how to recognize and minimize the hazards of working in the proximity of overhead power lines.
