Text For Citation: 01 Item/Group: 002B Hazard:
29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: a) Baler Area, 4581 Lower Valley Road, Atglen, PA 19310: The potential energy created by paper or books accumulating above the Fluffer Baler when it becomes jammed or clogged was not rendered safe for employees engaged in maintenance procedures for the Fluffer Baler. Employees occasionally open the lower door of the Fluffer Baler in order to unclog the equipment and the lockout procedure did not include a means to eliminate the hazard created by falling paper. On March 20, 2015, one employee was engulfed and another employee had his hand injured when paper that was clogged in the Fluffer Baler released and dropped on the employees.
