Text For Citation: 01 Item/Group: 001B Hazard:
29 CFR 1910.269(d)(6): The established procedures for the application of energy control (the lockout or tagout procedures) shall include the following elements and actions, and these procedures shall be performed in the following sequence: On March 14, 2015 the employer directed its contractor to remove electrical insulators in close proximity to energized parts in Cubicle 9, Bus 12, and was not required to perform the following steps before starting work: (1) know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy, (2) turn off and shut down the machines or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device, (5) prior to starting work on machines or equipment that have been locked out or tagged out, verify that isolation and de-energization of the machine or equipment have been accomplished. The employee was required to rely upon the system operator and was exposed to electrocution/high voltage electric shock.
