Text For Citation: 01 Item/Group: 002 Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: On or prior to 2/28/89, the employer failed to apply sufficiently designed equipment in that a model BR-31 Hi-Velocity Discharge Silencer was installed with a stepdown exhaust port and a closed valve, located in a brine well located at 1501 Homewood Ave., Barberton, Ohio, thus exposing the employee to serious injuries such as: fractures, lacerations and/or death. One feasible and acceptable method among others to correct this hazard would be the following: (1) The use of equipment sufficiently designed to withstand the maximum pressure encountered and (2) Train employees in the safe use of the equipment.
