Text For Citation: 01 Item/Group: 007 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to serious injuries from falls while working from elevated areas where a safe platform was not provided to work from: Among other methods, one feasible and acceptable method to correct this hazard is to provide work platforms at areas where employees are required to work at elevated areas: In the following instances, work platforms were not provided for employees working at elevated areas: a. BOND MILL #3 PENT HOUSE - The oiler is exposed to fall hazards when he has to climb on the guard to grease the bearings. There was no platform and guardrails provided. b. BOND MILL #3 PENT HOUSE - The oiler is exposed to fall hazards when he has to walk on open steel to grease the bearings of Fan 153. There was no platform and guardrails provided. c. #6PM BASEMENT 2ND PRESS - One employee was exposed to a fall hazard of approximately 8' while working from a felt roll during the felt change without fall protection. There was no platform and guardrails provided. d. BOND MILL #4 PENT HOUSE - The oiler is exposed to fall hazards when he has to climb on the motors of the #4 MC Exhaust Fan No.174 and the No.3 Supply Fan No.169. There was no platform and guardrails provided. e. #5PM BASEMENT DRIVE SIDE FAN PUMP- A platform and guard- rails were not provided for oiler to adjust cooling water for bearings of pump. f. #1PM OPERATING FLOOR TENDING SIDE - A platform and guardrails were not provided for an employee to scrape the crumb catcher approximately 5' from the floor. g. #7PM OPERATING FLOOR, DRIVE SIDE FOURDRININER - No platform and guardrails were provided for employees who have to climb out to remove retainers. h. #8PM OPERATING FLOOR, UPPER PRESS LEVEL - Though a 4' access ladder was provided, no platform and guardrails were provided for the employee to stand on while manually adjusting the automatic palm guide adjacent to the moving felt. i. #1PM OPERATING FLOOR, DRIVE SIDE FOURDRINIER - Employee was not provided with a platform and guardrails over the flat box suction hoses in order to adjust the squirters. j. #1PM OPERATING FLOOR DRIVE SIDE PRESS AREA - Employee was not provided with platform and guardrails to scrape the crumb catcher approximately 5' above the floor.
