29 CFR 1910.212(a): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about February 1, 2015 , at 5695 Davinci Lane , Carthage Missouri, employees were exposed to serious injuries when working at the Vemag machine on line 4, where high revolution shafts were not covered or guarded to prevent rotating parts from contacting the operator. (b) On or about February 1, 2015 , at 5695 Davinci Lane , Carthage Missouri, employees were exposed to serious injuries when working at the Batter applicator Stein DSI XL-A series bread applicator on line 4, where high revolution shafts were not covered or guarded to prevent rotating parts from contacting the operator. Abatement certification for this item is required; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
