Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: On or about September 26,1995, and at other times, at the Xenia McDonald's restaurant, employees were exposed to the hazards of acute intoxication and asphyxiation due to leaking hose connections of the carbon dioxide fill system at the Xenia McDonald's restaurant. Feasible means, among others, to abate this hazard would be to properly inspect, and maintain the carbon dioxide hose connectors and valves and the receiver connections to ensure proper sealing of the connections and to develop specific procedures to inspect connector mechanisms that have been frozen due to the presence of carbon dioxide. Note: This inspection found that this condition may also exist at other McDonalds locations and checks of these other facilities, with appropriate followup action is recommended.
