Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) On or about January 28th, 2015, employees operating, maintaining and/or servicing machines in the production areas of the plant were exposed to potential injuries from unexpected machine start-ups in that the employer had not developed clear and specific procedural steps for isolating each machine from its energy source(s). These machines included, but were not necessarily limited to, the "Panel Glue Wheel" machine and the "Raimann Gang Rip" machine in the Mill Shop. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
