29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. On or about 29 January, 2015, employees were engaged in masonry work while working from a six (6) foot scaffold that did not have proper means of access. Employees were using the end frame of the scaffold to gain access to the scaffold platform exposing them to a fall from an elevation of ~6 feet to the concrete surface below.
