Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causingor likely to cause death or serious physical harm to employees in that employees were exposed to: The hazards of severe lacerations, avulsions, fractures, and death from high velocity flying machine parts while operating and/or working near the Versa cutter Model 45W, which was run at rotational speeds in excess of those established by the manufacturer, on which 2 of six knife clamping righ screws were missing, and on which the employer had performed, or caused to be performed, welding of the clamping ring without consulting with the manufacturer. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Operate the Versa cutter only within the speed range specified by the manufacturer, i.e.: 1,500 RPM maximum; 2) Repair or replace all broken, damaged, or worn parts with parts at least equivalent to those supplied by the manufacturer; and 3) Consult with the manufacturer before effecting any modifications or repairs that could adversely affect the safety of the operation.
