Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The hazard of being crushed between railroad cars. On or about December 13, 1989, at the west end of the middle track south of the furnace, employees attempted to couple railcar #44 to railcar #30 which was coupled to two other cars pushed by a switch engine. The chain connecting the pin lifter rod to the pin was broken and missing rendering teh coupler device inoperable. An employee had to enter between the two railcars and manually open the defective coupler. Car #44, sitting on an incline, rolled back toward the employee between the two cars. Among other methods, one feasible and acceptable abatement method to correct this hazard is to maintain couplers in operable condition and to enforce employer's job safety analysis policy that reads: Scotch uncoupled cars - don't step between moving cars.
