Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of being struck by or caught in the silo conveyor or turn head mechanisms due to inadvertent and unintentional activation of silo and hopper conveyors located at 11470 North Fulton Industrial, Alpharetta, Georgia. Chain and sprocket and overflow in silos are such that employees were required to climb onto framework to reach maintenance points on or about 1-12-88. Among other methods, one feasible and acceptable method to correct these hazards is to lock or tag out prime movers or powered accessories with a formalized procedure designed to protect all persons or groups involved with the conveyor against unexpected operation.
