29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes, more than six feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) 610 N Bodine St, Philadelphia, PA - The employer did not ensure that each employee was protected from falling through the hole in the floor on the walking/working surface by installing some type of fall protection system. This hazard exposed the employees to a fall hazard. This condition was observed on or about November 20, 2014. The WVM Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.501(b)(4), which was contained in OSHA inspection number 978971, citation number 1, item number 1 and was affirmed as a final order on October 24, 2014, with respect to a workplace located at 1535 Cecil B. Moore Ave, Philadelphia, PA.
