29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.452(c): a) On or about November 20, 2014; trench excavation NE corner of CR-13 & Hwy-64, Daphne, Al: The employer failed to ensure employees were properly protected while connecting new sewer lines 5'4" and 5' 8" in depth. b) On or about November 20, 2014; excavation NE side of CR-13, Daphne, Al: The employer failed to ensure proper benching was done on the West side of the excavation which was 8' deep and containing a gas line, exposing employees to the hazards associated with cave-ins. c) On or about November 20, 2014; excavation NE side of CR-13, Daphne, Al: The employer failed to ensure proper protection was provided for an employee getting in/out on the South end of the excavation which was 8' deep, exposing him to the hazards associated with cave-ins.
