Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) An employee situated on the walking beam of a Churchill pumping unit was crushed between the horse's head of the pumping unit and the tubing board of a workover rig while attempting to remove the horse's head without proper measures having been taken to prevent movement of the walking beam. Among other methods, one feasible and acceptable abatement method to correct this hazard is to secure the counterweight and crank or walking beam of he pumping units with chains before working on the units or other acceptable means to prevent movement.
