Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing of likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of fire and/or explosion which could result in fatal injury: a) On or about 9/21/88 - the #1, Inductor-f50 Induction Furnace electrical powered cabinet located at the casting area of the K & D Building, the #1 Trippler Frequency Multiplier malfunctioned and fatally injured the operator during the production of copper alloy. Incomplete maintenance of the cooling system may have contributed to the accident. One feasible and acceptable abatement method among others to correct this hazard is: 1) To have qualified personnel perform maintenance on this equipment in accordance with manufacturers specifications. 2) To maintain operator logs that will record all deviations and problems experienced while operating the equipment. This is necessary to evaluate the system performance for a good preventative maintenance program.
