The alleged violations below have been grouped because they involve %% similar or related hazards that may increase the potential for injury resulting from an accident. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Drilling site located near Gilcrest, CO within Section 16, Township 317, Range 65 West - On or about August 6, 1991 and at times prior thereto, the employer did not ensure that each employee working on Rig #3 received Blowout Preventer Classes, did not ensure that employee(s) were trained to recognize the need to test the Hydril upon installation and during each round trip, did not ensure that employee(s) were trained in necessary inspection and maintenance procedures for the Hydril, and did not ensure that training regarding how to operate the blowout preventer was provided to each employee. Among other methods, feasible and acceptable methods to correct these hazards are: Ensure that each employee receives blowout preventer training, ensure that employees are trained to recognize the need to test the Hydril upon installation and during each round trip, ensure that employees receive training in necessary inspection and maintenance procedures for the Hydril, and ensure that training is provided to each employ- ee regarding how to operate the blowout preventer used on the rig.
