Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazads that were causing or likely to cause death or serious physical harm to employees in that: a) Saint Vincent DePaul Thrift Store, Elevator No. 2: On or about 9/16/89, and at times prior thereto employees were exposed to the hazard of being crushed by the moving elevator car and to falls down the elevator hoistway. Among other methods, feasible and acceptable abatment methods to correct the hazards are: 1) All hoistway entrances must be provided with hoistway doors or gates which guard the full width of the hoistway to a height of at least 6 feet. The hoistway doors or gates must be provided with mechanical locks and electrical controls or with interlocks which will prevent movement of the car unless all gates or doors are in the closed position and will prevent the gates or doors from being opened from the hoistway side unless the car is at the landing. 2) The elevator car must be provided with a car door or gate which guards the full width of the car to a height of at least 6 feet. The car door or gate must be provided with an electrical contact that will prevent the operation of the elevator unless the door or gate is in the closed position. 3) An emergency stop switch, that is manually operated, red in color, and conspicuously marked "STOP" must be provided in the elevator car adjacent to the operating control. 4) The elevator car top must be capable of sustaining a load of at least 300 pounds on any square area 2 feet on a side. 5) The elevator car and the counterweight must be provided with buffers. 6) A stop switch must be provided in the elevator pit. 7) A capacity plate, giving the rated capacity of the elevator, must be provided in the car. 8) The car light must be repaired and made operable. 9) The final terminal stopping switches located in the elevator pit must be repaired and made operable. 10) All debris should be removed from the elevator pit. 11) The elevator car safety must be provided with a switch which, when operated, will remove power to the elevator before or at the time the safety is applied. 12) The elevator landings must be so lighted that the illumination at the landing sills, when the elevator is in service, is not less than 5 foot candles. 13) The elevator must be placed under a maintenance contract with a qualified service company, so the all systems that assure safe operation of the elevator will be maintained in proper working order so that they will perform the functions for which they are intended. ABATEMENT NOTE: An action plan for correcting the above items must be submitted in the Billings Area Office on or before 10/5/89.
