Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a) Saint Vincent Depaul Thrift Store, Elevator No. 1: On or about 9/16/89, and at times prior thereto employees were exposed to the hazard of being crushed by the moving elevator car and to falls down the elevator hoistway. Among other methods, feasible and acceptable abatement to correct the hazards are: 1) All hoistway entrances must be provided with hoistway doors or gates which guard the full width of the hoistway to a height of at least 6 feet. The hoistway doors or gates must be provided with mechanical locks and electrical controls or with interlocks which will prevent movement of the car unless all gates or doors are in the closed position and will prevent the gates or doors from being opened from the hoistway side unless the car is at that landing. 2) The elevator car must be provided with a car door or gate which guards the full width of the car to a height of at least 6 feet. The car door or gate must be provided with an electrical contact that will prevent the operation of the elevator unless the door or gate is in the closed position. 3) The V-belt drive system is prohibited and must be replaced with a direct drive system. 4) A metal of concrete floor, capable of supporting at least a 300 pound load on any 4 square inches, shall be provided at the top of the elevator hoistway. 5) The elevator must be provided with a pit of proper depth for the car speed and design. 6) The elevator car must be provided with proper top and bottom clearances for the car speed and design. 7) The elevator can and counterweight must be provided with buffers. 8) The elevator car must be provided with a proper electric light. 9) The elevator car must be provided with a car safety device of a type proper for the car's speed and operation. 10) The elevator car safety must be provided with a switch which, when operated, will remove power to the elevator before or at the time the safety is applied. 11) The elevator car safety device must be activated by a speed governor. 12) The elevator's winding drum machine must have a final terminal stopping switch located on and operated by the driving machine. 13) The elevator must be provided with final terminal stopping switch located in the hoistway and operated by cams attached to the car. 14) The elevator's suspension ropes must have the drum ends of the ropes secured on the inside of the drum by clamps or by tapered babbitted sockets or by other acceptable means. 15) The counterweight ends of the counterweight wire ropes must be fostered by individual tapered babbitted rope sockets or by other acceptable means. 16) An emergency stop switch, that is manually operated, red in color, and conspicuously marked "STOP" must be provided in the elevator car adjacent to the operating control. 17) The elevator car and the counterweight must be provided with buffers. 18) A stop switch must be provided in the elevator pit. 19) The elevator landings must be so lighted that the illumination at the landing sills, when the elevator is in serivce, in not less than 5 foot candles. ABATEMENT NOTE: Until the above listed items are completely abated, this elevator must be placed out of service with all hoistway gates closed and with the fire enclosure doors locked.
