29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) & (c): During instances including, but not limited to the following, the employer did not ensure that appropriate cave-in protection was used: On or about 9/26/11 during the installation of a gas main, the employer did not ensure employees were protected from a cave in while working in an excavation that measured 4.7 to 7 feet in depth, 25 feet in length, and 10 to 11 feet in width. 1127 CONSTRUCTION INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD CFR 1926.652(A)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312965833, CITATION NUMBER 01, ITEM NUMBER 02C AND WAS AFFIRMED AS A FINAL ORDER ON 03/24/09, WITH RESPECT TO A WORKPLACE LOCATED AT HINES HILL & RTE 8 CUYAHOGA FALLS, OHIO 44223.
