29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a. On or about November 6, 2019, in front of the residential building, an employee operating a mortar mixer was exposed to amputation hazards and was injured. The work related incident resulted in amputation and the employer did not report it to OSHA within 24-hours of occurring.
