29 CFR 1904.29(a): A log of all Work-Related Injury and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injury and Illnesses, (OSHA Form 300-A), and/or the Injury and Illness Report (OSHA Form 301) or equivalent forms were not kept by the establishment. On or about June 8, 2018 and June 13, 2018, the employer did not maintain OSHA 300s (OSHA Logs) and OSHA 300A's (Summaries) of all work-related injury and illnesses for the calendar years of 2018; including two recordable injuries occurring on June 1, 2018 and June 6, 2018 respectively. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
