29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours. a. On or about May 15, 2017, at Goodwill Industries Big Bend, Inc. the employer did not provide OSHA 300 logs or equivalent logs within four business hours of requesting them to an authorized representative. Goodwill Industries Big Bend, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.29(a), which was contained in OSHA inspection number 1064046, citation number 01 item number 01 and was affirmed as a final order on July 3, 2016, with respect to a workplace located at 143 N. John Sims Parkway, Valparaiso, FL 32580.
