29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification. a) Jobsite - Employees were exposed to shock hazards from job made electrical extension cords that were not installed according to the instruction included in the listing and labeling. Extension cord was used to power a Red Arc Welder. Observed on or about 29 January 2016.
