29 CFR 1910.178(q)(6): Industrial trucks were altered by the addition of extra parts not provided by the manufacturer. Additional counterweighting of fork trucks was not approved by the truck manufacturer: a) In Shop 3, on Aug. 4, 2014 and at times prior thereto, forklift operators used the 1995 Hyster H100XL LPG forklift (Serial number G005D05710S) upon which an additional counter weight had been added without manufacturer approval and modification of capacity. Note: Abatement certification AND documentation are required for this item.
