29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity): or in the alternative 29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: (a) On or about January 05, 2023 and prior - Building 4, employees were exposed to fall hazards of up to approximately 30 feet when guardrails were not installed or were partially installed on the ends of the outrigger/sidearm platform between the third and fourth levels of the scaffold on the front of building 4, on the ends of the third and fourth levels of the scaffold on the front of building 4, on the ends of the fourth level of the scaffold at the corner of building 4 on the front of the building (used to access the scaffold from the third floor balcony), and on the ends of the fifth level of the scaffold at the corner on the front and end of building 4 that were connected with a crossover.
