The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: On or about March 3, 2016, the employer had not documented as required the powered industrial truck training that was provided for the operator of the electric and propane riding fork truck(s)in use on site.
