At the site, employees were exposed to fire and explosion hazards during cylinder refurbishing and filling operations.: Operating procedures were not implemented and developed to ensure that leaking cylinders and/or cylinder valves were handled in a safe manner and at a designated safe location, and that the sorting of incoming cylinders to the process was handled such that any residual propane in the cylinders was detected and evacuated in a safe manner.
