29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Jobsite - Monarch Brands attached a Cascade Brand Clamp System to a Yale Brand Sit-Down Forklift, Model GLC030VXN (SN#C809V04848G) affecting the capacity load and height of the powered industrial truck. Monarch Brands did not have prior approval from the manufacturer to make the modifications. The truck was not marked to include the load capacity information for the attachment. Observed on or about 19 OCT 2016.
