29 CFR 1910.146(d)(8): Under the permit space program required by paragraph (c)(4) of this section, the employer did not designate the persons who were to have active roles (as, for example, authorized entrants, attendants, entry supervisors, or persons who test or monitor the atmosphere in a permit space) in entry operations: a) At 4350 Semple Avenue in Saint Louis, MO 63120 - On or about 07/24/2015, the employer failed to designate the person(s) who were to have the active role as entry supervisor when employees were entering into a permit required confined space, "North Vapo Pit Basement," where oxygen deficiency, mechanical, electric, and engulfment hazards could be present.
