Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device prior to working on the machine or equipment: a) EAF system: On or about June 2, 2014 and times prior thereto, the employer did not ensure the authorized employees affixed a personal lockout and/or tagout device to the host sites lockout of the EAF system while conducting the contracted services which exposed employees to thermal and chemical hazards associated with the hot EAF dust, hot gases, and hot air if EAF system started while conducting services in the drop out box.
